Here is a statistical quandary that occurred to me when I woke this morning. The World Health Organization lists the stillbirth rate in the United States for 2009 at 3/1000 (Ranked 17th out of 193 countries). The study by Grunebaum, et al found that 83.2% of the births (13,891,274/16,693,978) in the U.S. from 2007-2010 met its criteria for greater than 37 weeks and 2500 gram singletons. They also state that greater than 99% of births were in the hospital. Making the assumption that all stillbirths have a 5-minute Apgar of 0 and multiplying the U.S. stillbirth rate of 3/1000 by .832 gives an adjusted stillbirth rate in the study population of about 2.8/1000 expected in hospital births. Yet in the study in question the 5-minute Apgar rate of 0 for hospital births is only 0.16/1000. That would be 16.8 times below the expected stillbirth rate. Since this study does not exclude known stillbirths I am totally confused as to how this discrepancy can be explained. Surely its not from the 0.58% of home births. Hmmmm, wouldn't it be interesting to know?
